Multidisciplinary Team<\/h4>\n          <p>Beaufort Family Practice is in partnership with the Ballarat Community Health Mental Health Multidisciplinary Team (BCH MH MDT) Program.<\/p>\n          <p><strong>Supporting people closer to home:<\/strong> The MH MDT Team provides coordinated, multidisciplinary mental health care for people with severe and\/or complex mental health needs across the Wimmera Grampians region.<\/p>\n          <p>The service works alongside General Practice and existing health and community services, providing assessment, treatment and care coordination while strengthening connections between the people and services already involved in a person&rsquo;s care.<\/p>\n          <p>BCH works with GPs to support shared care, improve communication between providers and reduce fragmentation.<\/p>\n          <p>Some assessment can commence by telephone or telehealth, allowing care to begin without waiting for the next regional face-to-face clinic.<\/p>\n          <p>BCH Mental Health Clinicians provide regular face-to-face services across the Wimmera Grampians region, supported by telephone and telehealth between regional visits.<\/p>\n
